How Can I Obtain a Copy of My Traffic Accident Report from the Illinois State Police?
You may request a crash report from the Illinois State Police online or by mail.
Logon to the Official ISP Crash Report web page, and be prepared to provide the following details from the accident:
A driver's first and last name
The date of the crash
The location of the crash, including the Interstate and County
The Agency Crash Report Number (located in the upper-middle portion of the motorist report)
Once the relevant information is entered, you can choose to have the accident report emailed or mailed to you by making the appropriate selection.
There is a $5 crash report fee, and all applicable processing fees must be paid before completing the request.
To request a copy of your crash report in writing, by mail, compose a letter listing the same details that are requested in the online form, including:

Logon to the Official CPD Crash Report web page, and be prepared to provide the following details from the accident, along with payment totaling $6 ($5 fee for the report, and $1 convenience fee):
The driver's name
The location of the crash
The RD number (report number given by the Police at the scene)
